*The Uniting and Strengthening America by Providing Appropriate Tools Required to Intercept and Obstruct Terrorism (USA PATRIOT) Act of 2001 requires financial institutions to take steps to obtain and verify certain identifying information for any person opening an account; maintain records of the information used to verify the person's identity; and determine whether the person appears on any list of suspected terrorists provided by the Federal government.
Primary forms of identification: A valid, unexpired, State issued driver's license or nondriver identification card equivalent; a valid passport; United States Government identification; United States Military identification; A valid, unexpired alien registration card.
Secondary forms of identification: Social Security card; Voter registration card; Original, State issued birth certificate; Credit card; Bank card; State or local Government identification; Valid, current company identification; Police identification; Health insurance card..
